Gayco Now Provides Medicare Part D Medication Therapy Management (MTM)

Medicare Medication Therapy Management (MTM) Programs offer free services to eligible members of Medicare Part D drug plans. These services help make sure that medications are working to improve their members’ health.

Who is this for?

If you have residents who have a lot of medications, they might qualify for a program that helps them manage their prescriptions. Typically, these are residents with chronic health conditions like high blood pressure or diabetes that have at least three qualifying chronic health conditions, take eight or more maintenance Part D medications each month and spend at least $4,044 on Part D prescription medications each year. The enrollment is automatic and free services, voluntary, and doesn’t affect your residents drug coverage under Medicare.

How does it work?

The MTM review is conducted by a Gayco Pharmacist with the help of your facility staff.

At least four times a year, Gayco’s MTM team will review the list of prescriptions your residents are taking. We call this a Targeted Medication Review. We may reach out to your staff if we see any potential problems or to conduct a brief questionnaire to determine if your residents are having potentially medication-related problems. This review will determine how well medication are working, whether any medications are having side effects, if there are interactions between medications that are being taken and whether costs can be lowered.

At least once a year, we’ll offer to talk to your resident or staff about how to use their prescription medication safely. We call this a Comprehensive Medication Review. The conversation can happen over the phone and/or questionnaire. After we complete the review, we’ll send your resident or staff a plan to help you get the best results from your resident’s prescriptions. We call it a Medication Action Plan.
